Recommended Accident Review Procedures

Follow these steps to review an accident:

  • Step 1 – Review the details of the accident, injuries, property damages and safety violations.
  • Step 2 – Document the details by completing a written report. Include the cause of the accident, if it was preventable and whether it will affect the involved employee’s safety record. Also, include recommendations for preventing similar incidents in the future.
  • Step 3 – Distribute copies of the report to the employee involved and applicable management (company owner, supervisor, HR, etc.)
  • Step 4 – Compile a cumulative report of all accidents for the week/month and review it at a managers’ meeting.
  • Step 5 – Make recommendations for company-wide action to increase workplace safety and prevent similar accidents in the future.

Recommended Discipline Procedures

Follow these procedures if an employee is found to be at fault for an accident:

  • 1st incident – Give a written warning
  • 2nd incident – Give a written warning + have a supervisor shadow the employee for a day to assess procedures used and discuss any safety concerns
  • 3rd incident – Give a written warning + short-term suspension without pay
  • 4th incident – Terminate employment
